چکیده

• For many years it has been recognized that normal human subjects and normal dogs retain salt and water only transiently and fail to develop edema during the chronic administration of large doses of a sodium-retaining hormone such as desoxycorticosterone acetate ( DCA ). Similarly, some Addisonian patients retain Na for only a few days in response to prolonged administration of DCA or aldosterone. During a normal Na intake, Na retention occurs for the first one to two days of DCA injection in dogs and for five to ten days of treatment with DCA in human beings -• but, thereafter, Na excretion returns to normal. This phenomenon has been referred to as the "escape" from the sodiumretaining effects of the hormone. The mechanisms involved in this "escape" have been the subject of considerable research and discussion but the problem remains unresolved. In contrast to the failure of chronic Na retention to occur in the normal dog and in some Addisonian patients with large doses of DCA, adrenalectomized dogs with thoracic inferior vena cava constriction ° or with severe pulmonic stenosis show virtually complete Na retention and accumulate ascites for weeks to months during administration of large amounts of DCA. Following removal of the caval ligature in adrenalectomized dogs, Na retention failed to occur despite the administration of six times the minimal maintenance dose of DCA. The presence of a caval ligature or of changes associated with depressed

Adjustment to aldosterone or desoxycorticosterone acetate induced sodium retention in patients with Addison's disease.

Long term administration of aldosterone (1) or desoxycorticosterone acetate (DCA) (2) to normal subjects has been shown, in certain instances, to produce continued renal potassium loss but only transient sodium and chloride retention. This adjustment of the renal response to sodium-retaining adrenal steroids has been termed an "escape" (3) from the expected effects of these compounds, and is se...

Pathogenesis of salt retention in dogs with chronic bile-duct ligation.

1. The present study investigates the role of mineralocorticoids in the pathogenesis of salt retention and ascites in dogs with chronic ligation of the common bile duct (CBDL). 2. After CBDL the natriuretic response to an intravenous sodium load [0.9% sodium chloride solution (150 mmol/l): saline; 10% of body weight] was markedly depressed. Urinary sodium excretion was 285 +/- 62 vs 960 +/- 58 ...

Renal Fractional Excretion of Sodium in Relation to Arterial Blood Gas and Spirometric Parameters in Chronic Obstructive Pulmonary Disease

Introduction: Arterial gas derangement could change urinary sodium excretion in Chronic Obstructive Pulmonary Disease (COPD) patients. There are very few and conflicting data in regards to the measurement of fractional excretion of sodium in COPD patients. The main aim of this study was to assess the relationship between renal fractional excretion of sodium(FeNa) with arterial blood gas and spir...

Clinical pharmacology of diuretic agents with special reference to chlorothiazide (diuril).

The factors responsible for congestive heart failure as a clinical syndrome may be multiple. Although decreased cardiac output relative to inflow produces a moderate increase in venous pressure, there is a concomitant decrease in renal blood flow which may contribute to a decreased renal excretion of sodium. There is also a decrease in hepatic inactivation ( ? due to congestion) of adrenal cort...

Metabolism of salt-retaining hormone by surviving liver slices.

Evidence has been produced in this and other laboratories that the impaired capacity to excrete sodium which characterizes a number of clinical states involving edema, ascites and anasarca, is mediated, in part at least, through a humoral factor emanating, in all probability, from the adrenal cortex (1-8). It now appears almost certain, on the basis of available evidence, that this substance, g...
